How do I get from Bangkok airport to Phuket, Chiang Mai, or Koh Samui?

Domestic flights are fastest and often cheapest: BKK→HKT (Phuket) 80 min ~฿1,500, BKK→CNX (Chiang Mai) 75 min ~฿1,200, BKK→USM (Koh Samui) 70 min ~฿3,500. Buses/trains are 10x slower but ~฿500.
